	Extended suspension/Expulsion form completed
	Copy of suspension letter to parent (without a return date)
	Copy of attendance
	Copy of discipline records
	Copy of report cards
	If Special Education student include copy of (typed) manifestation determination
	Memo/report to appropriate superintendent from principal (including principal’s recommendation)
	If applicable, copy of the police report
